The tourniquet controversy.

چکیده
Describing a tourniquet as “an instrument of the devil that sometimes saves a life” encapsulates the considerable risk to a limb when a tourniquet is applied to arrest life-threatening extremity hemorrhage. The use of tourniquets is widespread in both military and civilian environments, particularly in the developing world; however, the balance of risk is unclear, and its efficacy is controversial and unduly influenced by folklore and dramatic Hollywood images. The tourniquet controversy remains unresolved and has not, to our knowledge, reached the pages of medical journals since 1940. The clinical questions that remain unanswered are as follows:

Tourniquets: a review of current use with proposals for expanded prehospital use.
The use of arterial tourniquets in prehospital emergency care has been fraught with controversy and superstition for many years despite the potential utility of these tools. This review examines this controversy in the context of the history of the tourniquet as well as its recent use in surgery and modern battlefield casualty care. Safe prehospital tourniquet use is widespread in the military ...
متن کاملTourniquet-Related Iatrogenic Femoral Nerve Palsy after Knee Surgery: Case Report and Review of the Literature
Purpose. Tourniquet-induced nerve injuries have been reported in the literature, but even if electromyography abnormalities in knee surgery are frequent, only two cases of permanent femoral nerve palsies have been reported, both after prolonged tourniquet time. We report a case of tourniquet-related permanent femoral nerve palsy after knee surgery. Case Report. We report a case of a 58-year-old...
متن کاملThe Return of Tourniquets
Uncontrolled hemorrhage from isolated penetrating extremity wounds can result in 100% mortality, but significant controversy still surrounds tourniquet use in civilian prehospital care. It’s been proposed that 10% of combat deaths in the Vietnam War resulted from uncontrolled hemorrhage from extremity wounds; many of the deaths were due solely to ineffective field hemorrhage control methods.1 M...
متن کاملA Comparison between Single and Double Tourniquet Technique in Distal Upper Limb Orthopedic Surgeries with Intravenous Regional Anesthesia
Background: Several studies have put an effort to minimize the tourniquet pain and complications after conventionaldouble tourniquet intravenous regional anesthesia (IVRA). We expressed in our hypothesis that an upper arm singlewide tourniquet (ST) may serve a better clinical efficacy rather than the conventional upper arm double tourniquet (DT)in distal upper extremity surgeries. Methods: In th...
متن کاملThe Effect of the Silicone Ring Tourniquet and Standard Pneumatic Tourniquet on the Motor Nerve Conduction, Pain and Grip Strength in Healthy Volunteers
Background: The pneumatic tourniquet (PT) is routinely used in upper and lower limb operations by most orthopaedic surgeons. The silicone ring tourniquet (SRT) was introduced in clinical practice the last decade. Clinical as well comparative studies in volunteers concerning its safety and efficacy have been published. The aim of this study was to investigate the postoperative effect of the si...
